Problem: In developed nations, about one in four adults have at least two chronic conditions, and more than half of older adults have three or more chronic conditions. Almost half of all Medicare spending is directed at individuals with 3 or more chronic diseases. They require the attention of multiple healthcare providers or facilities and homecare for unique needs, disabilities, or functional limitations. Deterioration evolves from one chronic condition to another. However, most current medical practice and research are largely based on a single disease model, failing to account for the simultaneous presence of multiple conditions. Early detection of life threatening events and deterioration is needed for saving lives and costs, thus such patients need to be monitored continuously and comprehensively without time and location limits.
