Cardiopulmonary bypass CPB though difficult, at ≈ 18 °C to reduce CMRO₂ permitting a Total Circulatory Arrest TCA up to 68 mins, is very advantageous in all complex neurological operations.
Presently despite great advancements, with or without CPB with TCA, neurological interventions are still quite hazardous.
Tech 4X40-40, as an alternative to CPB with TCA, will tremendously facilitate Neurosurgery whilst avoiding morbidities and mortalities.
These patients with complex arteriovenous malformations AVMs or tumors are deeply anesthetized, intubated, transferred to cold rooms at 10 °C and immersed in circulating water at 4 °C:
This is Tech 4.
A femoral artery FA catheter and a femoral vein FV catheter are inserted after open exposure; the FA catheter is connected to a reservoir of Artificial Blood AB, a new preserving solution PS X40 at 4 °C, which contains the requisite amount of O₂, vasodilators, heparin, nutrients etc.
As soon as the patient fibrillates, 40 liters of AB are flushed to effect core-cooling followed by 4 liter per hour at 40 mm Hg pressure for 40 days to satisfy fully the patient’s O₂ requirement at 10 °C:
This is Tech 4X40-40.
Neurosurgeons can thus perform whatever complex interventions uneventfully and in optimum conditions followed by smooth post-op.
After 40 days, the patients are revived through cardiopulmonary bypass and respiratory support.
Possibly there will be 100 % survival and no morbidity with:
(1)All surgical wounds healed.
(2)No intracranial bleeding or edema.
