Sub mitral aneurysm is a rare cardiac entity initially described in African black patients .It seems to be due to a congenital weakness of the fibrous annulus at the level of the atrio-ventricular junction. Patients maybe entirely asymptomatic for many years or have varied clinical manifestations. We are presenting our retrospective review of surgical treatment of 20 patients at our institution during the periods 1998-2017. Patients where identified depending on the posterior mitral annulus involvement and variation in position of the respective necks involved. The surgical repair can be complicated at times with acceptable outcomes.
