A case of a 39 year old refugee who presented from a local clinic with exertion dyspnea and polycythemia. He went through a complete examination, which produced severe pulmonary hypertension on Echocardiography, peri-portal fibrosis on liver ultrasonography, and pulmonary arteries dilatation and fatty liver on CT angiography. It was concluded the most common diagnosis to be schistosomiasis. The case and characteristic radiological findings of schistosomiasis will be presented.
