Introduction: The urachal remnant typically undergoes obliteration in early infancy. When infected, it produces nonspecific symptoms, making diagnosis challenging.
Case Report: A 4-year-old healthy female patient, admitted to the ER several times, presented with constipation and periumbilical tender hematoma. An ultrasound examination showed a heterogeneous mass attached to the urinary bladder, raising suspicion of sarcoma. Combination of MRI, later biopsy, and follow-up ultrasound post Augmentin treatment, confirmed diagnosis of infected urachal remnant.
Conclusions: Urachal anomalies are rare and their presentation atypical, and may mimic malignant processes. Thus, a comprehensive radiologic evaluation and follow-up is necessary to achieve an accurate diagnosis.
