We present a case of a 73 year old man, with persistent nonproductive cough for several months and an obstructive pattern on PFT. While CXR was unremarkable, a CT revealed multiple sessile nodules protruding into the tracheal lumen. Imaging findings were consistent with BRONCHOPATHIA OSTEOCHONDROPLASTICA (TO), as confirmed by biopsy. TO is a rare idiopathic disease of the tracheobronchial tree and is commonly overlooked, just as with many of the other cases of tracheal pathologies. This case emphasizes the importance of routine evaluation of the tracheobronchial tree. Typical imaging findings of TO and the main differential diagnosis will be discussed.
