Chronic Exogenous Lipoid Pneumonia

An 85 y/o man was admitted to the ER with dyspnea. A PE protocol CT revealed bilateral PE with bilateral lower lobes consolidations.

On further anamnesis he confessed of shortness of breath and chronic cough for over a year.

Another chest CT demonstrated the same bilateral consolidations, with density measurements as fat. on BAL biopsy: Lipoid material, inflammatory cells and young fibroblasts.

Lipoid Pneumonia is a rare disease, which is caused by either aspiration of exogenous lipid material or endogenous source.

The diagnosis requires a high grade of suspicion. In our case the patient used nasal inhalers many years.
