Background: Streptococcus Constellatus (SC) is well known for its pyogenic infections especially in the abdomen.

Objectives: to report and characterize nine patients with SC appendicitis.

Methods: we report nine cases of SC appendicitis presented to the emergency room with typical symptoms of appendicitis.

Results: We had a total of 9 patients who were diagnosed with SC appendicitis three of whom had an accompanied peritonitis, mean age of 9.8±4.2 years old, eight of them were males with no apparent past medical history. The rate of symptoms at presentation was abdominal pain (100%), vomiting (77.8%), fever (22.2%), and diarrhea (22.2%).

Patients with SC appendicitis, have a severe clinical course with possibilities for failure on non-operative treatments. Higher rate of readmission 66% among those patient is noted.

Conclusions: Surgeons should be aware of the SC appendicitis and its severe clinical course, longer treatments with antimicrobial agents should be warranted in these patients.
