Application of ICF Framework to Feeding Disorder in Child with Cornelia De Lange Syndrome: A Case Report

Cornelia De Lange Syndrome is a genetic, multisystem developmental disorder that affects multiple systems in the body and manifests in diverse ways across individuals. Children with Cornelia De Lange Syndrome are more prone to experiencing feeding disorders and selective eating issues compared to typically developing children or those with autism spectrum disorders. Using the International Classification of Functioning, Disability, and Health (ICF) framework, this case report examines the feeding disorder in a three-year-and-eight-month-old boy with Cornelia De Lange Syndrome, focusing on identifying treatment goals and strategies.

The feeding assessment consisted of qualitative interviews, observation, and collection of Parenting Stress Index-Short Form. The analysis identified activities and participation as the primary barriers. Based on these findings, the therapist established the following treatment goals: 1) Increase the child’s food acceptance to at least two different textures and five different flavors and 2) Enhance the child’s adaptability to at least two types of feeding utensils.

Between April 14, 2023, and August 23, 2024, the child participated in weekly 30-minute therapy sessions, which included oral massage, desensitization techniques, shaping, and environmental adjustments. After thirty-seven sessions, the treatment goals were successfully achieved. This case report emphasizes the importance of a holistic approach in assessment and treatment, facilitated by the ICF framework. This approach not only enhances the child’s feeding behaviors but also alleviates family stress and elevates the quality of clinical care provided by the therapist.
